Service Agreement
This is the agreement that sets out how we'll work together. It can be completed by the participant, or by a family member, carer, plan nominee or enduring power of attorney (EPOA) on their behalf.
Before you start, a few things to know:
- It takes about five minutes. Have the NDIS plan handy for the plan number and dates.
- You can sign right on the screen with your finger, mouse or trackpad. No printing or scanning.
- When you finish, a copy is emailed to you automatically for your own records.
