Refer an older adult for home-visit physiotherapy. Care managers, families and home care providers are all welcome to use this form. We contact the client within one business day and send you an assessment summary.
Prefer to talk it through first? Call us on 0490 427 392, or email referrals@shinealliance.com.au. We bill Home Care Package providers directly, so there's no out-of-pocket cost to the client where the package has budget available.
