What a DVA home visit looks like

Your first visit is a thorough assessment: we listen to your history, look at how you're moving, and agree on goals that matter to you, whether that's mowing the lawn again, walking the dog, or just getting through the day with less pain. Every visit after that combines hands-on treatment with a practical exercise program, adjusted as you progress.

We keep your GP in the loop with progress reports, and when your treatment cycle ends we handle the paperwork for renewal so your care continues without gaps.

Cost: nothing out of pocket

For eligible veterans, DVA covers the full cost of home-visit physiotherapy, including our travel. We bill DVA directly. No gap fees, no invoices, no surprises. All you need is a current D904 referral from your GP.

DVA physio FAQs

Will I have anything to pay as a DVA client?

No. For eligible Gold Card holders, and White Card holders where physiotherapy relates to an accepted condition, we bill the Department of Veterans’ Affairs directly. There is no gap fee and no out-of-pocket cost to you, including for home visits.

What is a D904 referral and how do I get one?

A D904 is the standard referral form GPs use to refer DVA clients to allied health. Just book a normal appointment with your GP and ask them to refer you to Shine Alliance Mobile Physiotherapy for home-visit physio. Your GP can send it straight to us, then we call you to arrange the first visit.

How long does a DVA referral last?

A GP referral covers a treatment cycle of up to 12 sessions or one year, whichever comes first. If you need ongoing care after that, we send your GP a report and they can issue a new referral. We manage this process for you so care isn’t interrupted.

I have a White Card. Am I covered?

White Card holders are covered for physiotherapy related to their accepted conditions. If you’re not sure whether your condition is accepted, call us. We can help you check with DVA before your GP appointment.
